INSTALLATION CHECKLIST QF509B & QF510

This Admin Bulletin 191/2 replaces the original Admin Bulletin 191

1        With effect from the beginning of April 1999 EDP introduced the
                        Installation Check List                          Form QF509.

          This has now been amended and becomes
                        System Installation Check List           Form QF509B
                        WAN Installation Check List               Form QF510

         They are to be used from May 2001 onwards.

2       Samples are shown overleaf, they are an aide-memoire, designed to help you ensure that installations are completed
         correctly.

3       With every installation sent from GDC there will be a copy of the relevant form(s).
         Production are responsible for ensuring that a copy of QF509B/QF510 goes with each system/WAN equipment.

4       The installation engineer is responsible for completing the QF509B and or the QF510. To complete the QF509B/QF510; enter
         the Customer Name, Location, System or Equipment Type and Date at the top; and Print their name at the bottom. Then all
         that is required is a tick in the YES or NO box for each of the items; then Sign at the bottom.
         Any NO tick will require a more detailed comment on the back of the form.
         NOTE, for the WAN Installation Check List, QF510; unless the Installation Engineer visits/accesses both the Main and
         Remote Sites, there should be some ticks in the NO boxes.

5       When completed, the installation engineer is responsible for forwarding the QF509B and or the QF510, together with the
        Installation Certificate (IC); to the Customer Services Supervisor (CSS) The CSS is responsible for checking the
        QF509B/QF510/IC and ensuring that any relevant action is taken. The CSS is then to forward the QF509B/QF510/IC to GDC.
        GDC will then forward the QF509B / QF510 to Contracts at Beauchief Hall for final filing.
